Employee injured in fall, dies later of heart attack

On June 6, 1997, Employee #1 was preparing to disassemble the overhead pipes on some food processing equipment. Because she was short, she had to stand in an area adjacent to several pieces of equipment. These included a condensate steam pot, a pipe located near the floor that ran from the processing kettle to a product pump, and an unguarded rotating shaft and coupling for the product pump and motor assembly. Employee #1 was stretching upward to reach the overhead pipe when the pump activated. The sound startled her, and she slipped and fell. As Employee #1 landed, her foot became wedged under the rotating coupling and her upper thigh contacted the hot steam condensate pot. She was rescued and her injuries were surgically repaired, but she died of a heart attack on June 10, 1997.
