Investigation Summary
Employee #1 was in a 12 ft deep trench that measured 200 ft long, 2 ft wide at the bottom and about 5 ft wide at the top. He was lining up and attaching 8 in. sewage pipe lines when the wall of the trench directly above him gave way, burying him under about 2 ft of dirt. Rescue efforts by coworkers were unsuccessful. Employee #1 was pronounced dead on arrival at the local hospital.
