Event: 10/23/1995
Employee killed in fall from roof

A roofing crew set a hoist on a roof and used a water barrel as the base counter ballast. A motorized hoist was loaded and operated so that the hoist would pull itself up the cable. Guy lines were used to keep the motor from striking the building. As the motor approached the cap of the roof, the boom was observed to dip and the motor started down. Two workers on the ground left the area to avoid being struck by the falling motor. As they turned to observe what happened, they realized the hoist frame had toppled down, knocking Employee #1 from the roof. He died of multiple injuries, including a concussion, sustained after being struck by the hoist and falling approximately 25 feet.
