Investigation Summary
Employee #1, a head repairman, was replacing an approximately 8 ft high by 12 ft long water cooling panel on a basic oxygen furnace (BOF) vessel. To access the panel, he placed a ladder on an 8 in. diameter pipe. Employee #1 was attempting either to gain access to the panel or to secure the ladder when he fell 22 ft to the ground. He sustained a blunt force trauma injury to his head, and was killed.
