Employee dies in tank trailer explosion

Employee #1 was performing a leak test on a 6,800 gallon tank trailer belonging to an oil recovery company. According to the written procedures, the tank was air compressed to approximately 1.6 psig. Evidence found at the scene revealed that Employee #1 was welding the closing device on the hatch. Once he struck an arc, the vapors within the tank ignited and the tank ruptured. Employee #1 was standing directly over the hatch when it ruptured and the blast propelled him approximately 20 to 25 feet through the roof. He landed on one of the roof beams and died.
