Investigation Summary

Investigation Nr: 846063
Event: 05/03/1996
Employee killed when struck by falling grizzly

At approximately 5:30 p.m. on May 3, 1996, Employee #1 was taking a break outside of the bagging plant, located at the northeastern corner of Waupaca Materials. A coworker operating a front end loader was transporting a 2 ton steel frame sorting device (grizzly) that was suspended on an elevated bucket. The operator lowered the grizzly to the ground, while still maintaining contact with the loader bucket. He then turned off the loader's engine, set the parking brake, and left the machine to retrieve a shovel to clean the grizzly's grates. He walked around to the front of the grizzly and started to scrape the debris from the grate. Employee #1 walked over to him, placing himself in front of the grizzly. The grizzly fell and struck Employee #1, who sustained crushing injuries to the upper body system. The operator, who was the only witness, heard a scraping of metal to metal and jumped to his right and clear of the falling grizzly. Employee #1 was pronounced dead at the scene by the local emergency rescue team.
