Warehouse Employee Is Killed in Fall from Order Picker

At approximately 10:30 p.m. on October 25, 2005, during the second shift, Employee #1 was working for the Boelter Companies, Inc., a wholesale distributor of alcoholic beverages. She was in a warehouse area, which had various storage racks and clear aisles that provided adequate space to move about and operate an order picker. She was operating a Caterpillar Model Number NOR30 order picker. She was at a height of approximately 12 feet. She was not wearing a belt or lanyard, and there were no railings in position to prevent her from falling. She fell approximately 12 feet, hitting her head on the concrete floor. She sustained a concussion to her head, and she was killed. There were no witnesses to the accident.
