Event: 10/22/1994

Employee is brain dead after fall from elevated platform

Employee #1 was installing electrical conduit from a platform raised by a forklift truck. While being lowered, the platform jerked or fell approximately 1 to 2 feet. Employee #1 was in a crouched position holding onto the top rail of the work platform. When the platform fell, Employee #1 fell from beneath the top rail approximately 10 feet and struck his head. (There was no mid-rail or toe board to prevent his fall.) He was declared brain dead and removed from life support 2 days after the accident. Employee #1 was an independent contractor hired to perform a specific job.
