Investigation Summary
Employee #1, of Quality Painters and Decorators, Inc., was using a spray gun to paint the steel supports in an impound area. He was working from a scissor lift elevated to approximately 11 feet. He fell off the lift and hit the concrete floor, suffering fatal head and chest injuries. It is assumed that he was reaching over to paint a spot that he missed, but there were no witnesses to the accident.
