Employee killed when crushed by loading shuttle

Employee #1 was unjamming a hot plate on a carousel-loading shuttle that was powered by a pneumatic cylinder. As he freed the hot plate, the stored energy in the pneumatic cylinder was released. The shuttle struck Employee #1 in his chest and abdomen and pushed him backward until his lower back was crushed against the carousel. He was killed.
