Employee crushed and killed in rock crushing unit

At approximately 3:00 p.m. on April 10, 1996, Employee #1 was working on a bucket conveyor leg replacing a couple of buckets. A truck driver who did not see the employee turned on the bucket leg so he could dump his load of sand. The employee did not have the machine locked out and was caught in the moving machinery. He was killed.
