Investigation Summary
At approximately 1:15 a.m., Employee #1, a yard switcher, was in the southwest corner of a terminal on the south side. He was between two 28 ft trailers cranking down dollies to one of them. Employee #1 had just finished cranking down the dollies when a coworker, also a yard switcher, began pulling out a 48 ft trailer that was parked in the southwest corner of the terminal on the west side. As the coworker began pulling out the trailer, its rear caught on the 48 ft trailer next to it, which in turn brought it out against the 28 ft trailers. Employee #1 was caught and crushed to death between the 28 ft trailers.
