Employee killed when pulled into belt conveyor

Employee #1, a maintenance employee, was working on the south foundry cool sanding elevator during shutdown operations within the plant. Tail pulley adjustments had to be made on the bucket-type belt of the sanding elevator and, for this process, an electrician energized the conveyor. At the same time, Employee #1 was pointing into the sanding elevator. His hand and then part of the upper extremities of his body were pulled into the belt conveyor. Employee #1 sustained multiple blunt force injuries and was killed.
