Employee killed in fall when rough terrain forklift topples

Employee #1 and a coworker were in a manbasket elevated to a height of approximately 35 ft on the forks of a Lull rough terrain forklift, installing bracing members to a cooling tower under construction. The workers were secured to the basket by full body harnesses and lanyards. For some reason, the forklift overturned and they rode the basket down as it fell. Employee #1's head struck against a concrete wall as the basket hit the ground, killing him. The coworker was not injured.
