Investigation Summary
Employee #1 was carrying a magnet frame with a forklift. The magnet frame was only being lifted by the right fork. Employee #1 was attempting a left turn when the mast of the forklift struck an overhead beam, causing the forklift to tip onto its right side. Employee #1 was pinned underneath the forklift and killed.
