Employee killed when caught in slitter stand

Employee #1, a roller foreman in the bar mill at a steel plant, was checking a slitter roll stand because of an unusual noise. According to coworkers, he was kicking the spindle box, which consisted of a set of rollers running at 380 rpm. Employee #1's foot became caught in the in-running nip point of the rollers and he was dragged into the machine up to his chest. Employee #1 was killed.
