Investigation Summary

Investigation Nr: 202085635
Event: 10/30/1998
Employee Crushed Under Roof Truss and Killed

Employee #1 was going up on a scissor lift to position one end of the conduit into a coupling. Another employee was on the ground holding onto one end of the conduit while Employee #1 attempted to turn the coupling onto the ends of the conduit. One end was hanging down from the rafters and the connection to be made was being held onto by both employees. Employee #1 was going up on the scissor lift to position the one end of the conduit into the coupling while the other employee was holding the other end in the air from the ground level. Employee #1 was attempting to make the connection but was not comfortable, could not properly reach the coupling, or there were possible problems with aligning the coupling with the existing drop. Employee #1 yelled down for the other employee to wait. The other employee brought the conduit down to rest on the floor while Employee #1 positioned the lift so he could get the coupling started on the pipe. The other employee stated that while positioning the pipe or conduit on the ground, he heard a gasping sound. He looked up and saw Employee #1 being crushed under the roof truss between the top rail of the lift and the bottom of the truss. Employee #1 was crushed and killed.
