Employee dies when struck in head by metal piece

At 8:40 a.m. on December 5, 1994, Employee #1 was flame cutting 35 inch square sections from a large forged steel cylinder, originally 150 inches in diameter by 130 inches high, about 2/5 inch thick, with no top or bottom. As he and a coworker neared the ends of converging horizontal cuts, the top portion of the metal sprang violently in their direction, striking Employee #1 in the right side of the head. A hard hat was in use, but failed under the diagonal blow. Employee #1 died.
