Investigation Summary
Employee #1 was refueling a portable forced-air heater while it was left running, a common practice. The fuel was transferred to the heater through a rubber hose that was attached to a pump. Apparently, the hose came in contact with flame discharge from the heater, which burned a hole in the side of the hose. Kerosene was then pumped out of the hole and was ignited by the heater. Employee #1 was killed by the ensuing fire.
