Employee Is Injured When Struck by Falling Pipe

At approximately 7:45 a.m. on May 16, 2007, Employee #1, a truck driver, was working with a coworker, a fork lift operator, loading six 6-inch ductile pipes, which were not secured, onto the back of a flatbed truck parked in the yard. The coworker was heading the forklift toward the truck from the side to unload the pipes. As the coworker got closer to the truck with the forks up, Employee #1 grabbed a strap from the strap box, and ran between the lift and the truck. The coworker thought that he would hit Employee #1 and applied the brakes, causing the front pipe to roll off of the tines and hit Employee #1 in the head. His head was pinned against the side of the truck bed, resulting in severe head injuries. He was hospitalized for his injury.
