Employee killed in fall from roof or scissor lift

Employee #1 and a coworker entered a scissor lift, and the coworker raised the lift until it reached the roof line. He then turned to talk to Employee #1, only to discover that he was no longer in the lift. Employee #1 had fallen approximately 29 ft onto the concrete floor, between the outside edge of the scissor lift platform and two downward bending roof decking panels. He died as a result of his injuries. There were no eyewitnesses, and it is unclear if Employee #1 fell from the roof or from the scissor lift.
