Employee Witnessed Fire and Explosion- No Injuries

Employee #1 witnessed a fire and explosion, but he was not injured. The incident did get major news media coverage. The fire and explosion occurred when an 8,250,000 gallon extractor tank heated carbolic oil above its flash point. An unused blanked off valve failed at its threads and allowed combustible liquids and vapors to escape the tank. The hot temperature in the tank ignited the vapors. The emergency vent on top of the tank could not be opened because the newly installed catwalk was placed directly on top of the vent to prevent the vent from opening. His employer failed to inspect and provide preventative maintenance of the old equipment. Employee #1 was not injured during the incident.
