Foot amputated when crushed by forklift

On April 9, 1999, a temporary employee was operating a stand-up-type powered industrial truck. He was backing the forklift up when he struck the rack guard. The employee's left foot was hanging out of the truck and was crushed between the rack and the truck. As a result of the accident, the employee's left foot was amputated.
