Employee killed when burned by molten plastic

At approximately 3:00 p.m. on February 21, 1996, Employee #1 was standing by a disassembled plastic extrusion machine that had not been locked out prior to maintenance. He was struck in the face and neck with thermal heated polyethylene. Employee #1 died as a result of burns sustained from the molten plastic.
