Employee falls 12 to 15 feet while operating order picker tr

At 7:15 a.m. on February 15, 2017, an employee working as a warehouseman (powered industrial truck operator) operating a Yale, Model OS030BF, Serial Number E826N01502K, High Lift Order Picker Rider Truck, known as OP3, to stock cases of wine on a storage rack. The OP3 order picker is a ride on type High Lift Order Picker Rider Truck. The order picker is operated by the operator standing on the truck operators platform. Forks are located behind the operator platform where pallets of material can be placed while stocking racks or building orders. While operating the order picker truck, the operator is required to use a fall restraint system attached to the overhead guard portion of the order picker. At the time of the incident the operator was raising the work platform approximately 12 to 15 feet in the air to stock a rack. While raising the platform the lift chains and lift chain anchor broke, causing the platform to fall to the warehouse floor. The operator was tied off using his fall restraint system at the time the platform fell. The operator rode the platform to the floor, resulting in fracture injuries to his right leg and ankle. An ambulance was called and the employee was taken to the hospital for medical treatment. As a result of the incident the employer removed the order picker from service and has since removed the order picker from the facility. The employer also replaced the lift chains on the other two similar order pickers used at the facility.
