Summary Nr: 93122.015 Event: 02/06/2017Employee Falls From Ladder And Fractures Leg
At 12:00 p.m. on February 6, 2017, an employee was climbing onto the rumbler sec tion of a Balcan Lamp Recycling Machine to unjam the hopper above the splitter f eed duct. As he stood on an 8 foot tall step ladder to access the top of the rum bler machine, the ladder fell from under him. The employee fell and struck the concrete floor below, resulting in a fracture of his lower left leg. The inciden t investigation revealed that the employee had not used the ladder as intended. Instead, he leaned the ladder against the machine without opening this ladder.
