At 12:00 p.m. on February 6, 2017, an employee was climbing onto the rumbler section of a Balcan Lamp Recycling Machine to unjam the hopper above the splitter feed duct. As he stood on an 8 foot tall step ladder to access the top of the rumbler machine, the ladder fell from under him. The employee fell and struck the concrete floor below, resulting in a fracture of his lower left leg. The incident investigation revealed that the employee had not used the ladder as intended. Instead, he leaned the ladder against the machine without opening this ladder. | ||||||
