Employee struck and killed by forklift

Employee #1 was leaning outside the protection of the roll bar cage on her forklift talking to the operator of another forklift. Both forklifts were moving. Employee #1 was struck on the head by the other forklift and fell onto its forks. She died from a severe skull injury.
