Employee dies in 10 foot fall from cherry picker

Employee #1 was pulling cases of merchandise from a Clark OPIS cherry picker 10 feet above a concrete floor. He stepped off the pallet on the cherry picker and slipped backward, falling to the concrete floor. He was wearing a safety belt, but it was not fastened to the cherry picker. Employee #1 died from severe trauma to the head.
