Employee dies of skull fracture after fall from ladder

At approximately 9:00 a.m. on December 19, 1994, Employee #1 was at a farm site using an auger to deliver feed from a bulk truck to the top of feed bins. He had used a ladder to access the top covers of the bins so that the auger could be inserted to fill them. Employee #1 had removed the covers from both bins and was filling one bin when he apparently fell from the ladder. The resulting skull fracture pierced his brain and he died.
