Investigation Summary
At approximately 1:00 p.m. on April 17, 2003, Employee #1 and coworkers were deburring a slab of metal using a grainer. Since the slab was heavy, the employee and a coworker were handling the piece from the front and two other coworkers caught the piece after it traveled through the grainer (piece was being deburred). While positioning and turning the slab for placement, the coworker dropped the slab of metal and the metal piece fell onto the moving conveyor, catching Employee #1's fingers between the metal and the top of the conveyor unit. The coworker pulled on the employee's finger and subsequently the finger tore off. Neither the emergency stop button, nor the bump strip in front of the unit was activated. Employee #1's finger was amputated and he was hospitalized.
