Event: 07/31/2003

Employee #1 was installing new equipment on cleaning process lines that carry a sodium hydroxide solution with a pH greater than 13. Employee #1 used the wrong pump that was rated at 25 psi. In order to make it work Employee #1 assembled the hoses with adapters and clamps. When Employee #1 hooked-up the hose to the rinse spigot that had 35 psi to 45 psi of pressure, backpressure built up in the pump. The hose was blown off the spigot causing the chemical to splash on both eyes. Employee #1 was fully protected, including safety glasses, and she immediately flushed her eyes.
