Investigation Summary
At approximately 5:00 p.m. on September 30, 2003, Employee #1 was on a vehicle being towed by a tractor. When the tractor operator backed up, Employee #1's left foot was run over by the rear tire of the machine. Employee #1 sustained a fracture to his foot and was hospitalized.
