Investigation Summary

Investigation Nr: 201485513
Event: 03/11/2003
Employee Amputates Toes from a Falling Steel Coil

On March 11, 2003, Employee #1 was removing steel coils that weighed 907 kilograms each. He unbanded each individual coil, then stood in front of the coils and cut the band securing the two coils together. An unseen piece of wood dunnage beneath it caused the rear coil to tip, pushing the front coil over and onto the employee's foot. The impact caused the amputation of 4 toes. Emergency medical personnel were summoned and transported him to the hospital. At the hospital, he underwent surgical operations to repair the fractured bones and damaged tissues in his foot. He had another toe amputated because of the poor circulation in the foot. After the surgeries, he was hospitalized for postoperative care and then released.
