Investigation Summary
Employee #1, of Warehouse Specialists, Inc., was opening the door on a freight car when a loose roll of paper tumbled out. The 2,200 lb roll had been stored at a height of approximately 10 ft. Employee #1 tried to run under it but became pinned against the loading dock. The roll then bounced up and Employee #1 fell to the ground. The roll came to rest on top of Employee #1. He was killed.
