Employee struck in face with roll bar

Employee #1, a welder, was using a 5 ft bar to turn over a 12 by 12 structural steel tube. The bar had a flat surface on the end with a notch into which 5/16 in. steel will fit so that it can be rolled over. Employee #1 put the bar on the vertical side and stood in front of it. He should have put the bar on the horizontal side and stood at its side. The bar slipped, hitting Employee #1 in the face and breaking his left cheek bone. He required hospitalization. Employee #1 was well trained at the job. No violations were cited.
