Investigation Summary

Investigation Nr: 170622591
Event: 08/19/1994
Employees exposed to ammonia leak from refrigeration system

At approximately 2:30 to 2:45 p.m. on August 19, 1994, a valve failure occurred on a refrigeration system. The door to the area was closed, the refrigeration system was shut down; the valve was changed; and the system was turned back on. At 4:00 p.m., Employees #2 through #7, involved in bread dough production, noticed ammonia leaking in the bread dough room. The area was closed off and the employees were moved to the office area. Employee #2 placed a fan (not explosion-proof) in the room to help move the air. The Fire Department was called. Sacramento County Environmental Management and the Sacramento County Fire Department's Hazardous Materials unit arrived and evacuated all employees from the building. Although Employees #3 through #7 were evaluated by EMTs and paramedics, none were taken to the hospital. No other employees were injured. Sacramento Refrigeration was brought in by the employer for a total system evaluation and repair as required by Sacramento County Hazardous Material. Several problem areas were found, which will take at least three to four days to repair.
