Investigation Summary

Investigation Nr: 201793643
Event: 09/14/2000
Employee injured when run over by pallet truck

At approximately 10:30 a.m. on September 14, 2000, Employee #1 was standing in front of his pallet truck Number 749. He was replenishing a selection bay filling fruit jars on Aisle 19 Bay 051 and as he was bending to cut plastic wrap he saw pallet truck Number 777 coming towards him. The operator of pallet truck Number 777 was traveling from the receiving area towards the shipping area by making a right turn around a corner and entering Aisle 19. As he made the right turn a pedestrian walked by, and he made a wider turn to avoid this pedestrian. He didn't notice Employee #1 until he had already turned and he tried to stop his pallet truck but was unable to due to its speed. Employee #1 tried to get out of the way but was struck on his right ankle crushing it. Employee #1 was hospitalized.
