Employee Falls from Rack

At approximately 10:00 a.m. on May 11, 2004, Employee #1 was climbing a rack to retrieve a box of product. The aisle between the racks was blocked with pallets, and the space between the rack and pallets was approximately 1 ft in width. The distance between each level of the rack was 56 in. in height. Employee #1 climbed over the pallets and onto the rack. He tried to reach a box of product on the fourth level of the rack while standing on the third level, but lost his balance and fell. He struck the ground, sustaining an injury. He was taken to a medical center where he was hospitalized for 4 days. As a result of the accident, Employee #1 had severe back pain, which radiated to his left leg and foot.
