Investigation Summary

Investigation Nr: 201115631
Event: 07/08/2003
Employee injured in fall from scaffold

At approximately 2:00 p.m. on July 8, 2003, Employee #1 was working from a 6-ft high rolling scaffold, placing dry wall on to the topside of the soffit. At the site in the interior atrium area, the employer had placed a temporary floor so employees could work on the upper soffit area of the dome. After placing the dry wall the employee was to mark and cut the dry wall to fit the rounded profile of the atrium. During the daily safety meeting employees received instruction on the hazards of working on a scaffold and with dry wall sheets. Employee #1, after placing the dry wall sheets on the upper soffit, stepped off the scaffold and placed his weight on to the metal frame supporting the dry wall. The portion of the dry wall that was to be trimmed off was not supported and broke off, causing Employee #1 to fall 8 ft to the floor. He was taken to Holy Cross Hospital for treatment of a bruised rib and released the same day. He was not hospitalized.
