Employee Is Killed When Installing Metal Sheeting

On March 15, 2005, Employee #1 and a coworker, workers for a company that constructs dairy buildings, were putting screws on metal sheeting that was located on the back of a metal wall structure that was under construction. As employee #1 reached for a screw gun that was located in a basket platform carried by a forklift that was traveling in reverse, he became pinned between the metal wall and the basket platform on the forklift. Emergency services were contacted, and Employee # 1 was pronounced dead by the Kern County Sheriffs Coroner with primary cause of death due to a serious head trauma. The accident investigation determined that as the forklift was backing up, its right front tire fell into a mud hole, causing the forklift operator to overlook the location of Employee #1. The employer was cited for several violations, including: the lack of the forklift operator having proper training certification and general practices observed.
