Employee's left finger partially amputated

On January 13, 2001, an employee for was operating a "Crimping Machine." The crimping machine swages one of the parts so that both parts become one part. A motor drive V-belt combo operates the crimper. The motor housing with V-belt and pulley is located below the table. She was trying to place the two part assembled cable into the machine. The V-belt and pulley was guarded. However, there was an opening in the guard two inches wide. A part of the cable got stuck on top of the machine. The employee reached under the table and placed her left pinky finger into the two inch hole to move the V-belt. When the employee freed the part, the V-belt immediately started to move catching her finger between V-belt and pulley. The crimping machine was fabricated by the company and had no manufacturer's plate other than the one on the motor. The employee suffered partial amputation of the left small phalanx. The employee was hospitalized for her injuries.
