Investigation Summary
Employee #1, a carpenter, and a coworker/foreman were working from ladders placed on the third floor of a building. Employee #1 had one foot on the ladder and the other on a wall support when he slipped and fell 7 ft to the second floor. He then went through the single guardrail and fell another 18 ft to the ground. Employee #1 suffered back injuries, for which he was hospitalized.
