Employee's fingers amputated in conveyor of transfer mill

At approximately 8:20 p.m. on August 18, 2000, Employee #1 was working near transfer mill #4. He had been working as a utility worker (a "jack of all trades") for a year and, at the time of the accident, was assigned to a cut-off machine next to the transfer mill. The assigned operator for the transfer machine was 15 ft from the control panel, performing some other task when the tubes on the transfer conveyor became jammed. Observing this, Employee #1 went to the machine, leaned over the conveyor, and tried to free the jammed tubes. In the process, he put his left hand in the path of the conveyor dog, catching and amputating his middle and ring fingers between the dog and a tube. Employee #1 failed to follow lockout/tagout procedures while clearing the jam.
