Investigation Summary
On October 9, 1998, Employee #1, a 19-year-old laborer, was working on a Commander power screen sieving machine. The power screen is a mobile, radial loading, high capacity stockpiling conveyor. Employee #1 was working at the loading end of the machine, where the flat conveyor belt originates movement powered by a 96 HP internal combustion engine. Power screen work started in the morning at 6:30 a.m. and after the operation had been underway for some time, the 3 ft wide flat belt was narrowed by 6 to 8 in. after a pipe with a sharp edge damaged the belt. This exposed some of the idlers and rollers under the belt to dust and other debris falling from it. Employee #1 was assigned to keep the idlers clean and to clear any jams of the elevated belt at the other end of the power screen. At approximately 5:30 p.m., he was cleaning the settled dust with his gloved hands and a poking rod when his left hand became caught in the nip point between the moving belt and a 4 in. diameter by 9 in. long idler. The force of the moving belt dragged his gloved hand, and then his forearm and upper arm, with great force. He began screaming but no one heard because he was the only worker assigned to the power screen machine. Several minutes later, a coworker passed by, raised an alarm, and stopped the machine. Other coworkers arrived and extricated Employee #1 by cutting the flat conveyor belt with a special purpose knife. He sustained extensive dirty lacerations, deep abrasions and contusions, and crushing injuries to his left arm, for which he was hospitalized approximately seven days. Employee #1 did not follow the established lockout/tagout procedure while cleaning the power screen, and was exposed to an unguarded nip point between the moving belt and idlers. Further, the manufacturer's recommendation was ignored when the width of the belt was modified and when the machine was not stopped before the guard doors were opened.
