Investigation Summary
At approximately 5:30 p.m. on August 18, 1997, Employee #1 was working inside the door #12 warehouse. She was using a rolling ladder to access boxes of merchandise on storage racks stacked 10 ft or higher from the floor. The rolling ladder had four wheels at the bottom and a lock near the two front wheels. When the lock was activated manually before ascending, the ladder was prevented from rolling. Employee #1 had climbed part-way up the ladder and was about to pick up a cardboard box containing a body pillow from overhead when the ladder lock apparently failed and the ladder moved. She lost her balance and fell 5 ft 6 in. to the floor and was knocked out. As she was regaining consciousness, her supervisor walked in and found her lying on the ground. Paramedics responded and transported Employee #1 to St. Mary's Trauma Center, where she was treated for head injuries. The cause of the accident was use of defective equipment. According to Employee #1, this was not the first time the ladder locks had failed.
