Investigation Summary
On July 18, 2007, Employee #1, a 19-year-old apprentice framer, and an experienced coworker, were on the perimeter of a 28 ft high roof, applying fascia boards to the ends of the 2 by 10 rafters of a roof section built-up to enclose insulation. One fascia board had a bow or crown in it and the two men were removing the crown before installing the board. Employee #1 was squatting inside the built-up section of roof, with the claw-end of his hammer on a screw-nail head driven into the fascia board. His intent was to apply force on the nail to remove as much of the crown as possible. Employee #1 was pushing hard on his hammer, applying force away from the structure, when the head of the screw-nail broke off. Employee #1 had too much of his mass outside the perimeter of the structure to recover his balance and he fell approximately 28 ft to the ground. He sustained serious injuries, for which he was hospitalized. The employer had not provided scaffolding or a catch platform beneath Employee #1 and the coworker. The employer also failed to provide personal fall arrest, personal fall restraint, or positioning systems.
