Mechanic's hand injured in chain and sprocket drive

Employee #1, a maintenance mechanic at Stone Products Corp., a manufacturer of cultured stone veneer, was standing on pallet line to adjust and re-install a chain-link pin that had worked itself out of the chain. The line had been shut down for the change of shift. The chain and sprocket drive on the mixing head above the hopper was on the backside of production line #2. The chain drive is 8 ft above floor level. The machinery had been custom-built by the company for this unique task. Employee #1 did not lock out machinery before working on the chain drive. The machinery was started by the line leadman who was unaware of his location and activities. Employee #1 caught his hand in the chain and sprocket drive, and sustained lacerations to his left hand, a fracture of his middle finger, and amputation of his index finger.
