Employee injured in fall from roof sheathing material

Employee #1 was framing the roof structure over the entryway of a single-story, detached residential unit. He was installing 2 by 4 rafters while standing 11 ft above a concrete deck on a cantilevered, unsupported 2 ft by 2 ft section of 1/2 in. thick chipboard roof sheathing material. The chipboard roof sheathing failed, bending at the outer edge of the outboard supporting board. Employee #1 fell and sustained fractures to his left elbow, left iliac crest, and transverse process L3, and a cut on his left eyebrow.
