Investigation Summary
A power line crew was repairing an overhead power line that had been damaged in a storm. The crew foreman was in the bucket of a one-person bucket aerial lift to install the power line on a utility pole. A power line worker was preparing to hoist the downed line to the foreman. The 12-kilovolt power line was deenergized when the crew arrived. However, the crew did not install protective grounds, nor did they place tags on the disconnection point. A troubleman was sent to the general area in response to outage reports. When he found no tags at the disconnection point, the troubleman reenergized the line to see if the fault was still present. When the line was reenergized, the power line worker, who was touching the power line, received an electric shock. He was hospitalized with burns up to third-degree on his left arm and right hand.
