Employee's finger amputated by unguarded saw blade

Employee #1, who had been employed by the company for 23 years, was operating a Rockwell table saw, model 1214. After cutting a piece of wood, he attempted to reach between the rip fence and the blade to remove scrap. His right index finger was amputated between the first and second knuckle. Although the area and the machine were generally clean and tidy, the blade guard stuck and was inoperative.
